Jesse Trevino Insurance Agency, Inc.
is an independent full-service insurance agency providing
a complete line of insurance products including life and health
products for individuals and groups; personal lines insurance
(auto liability and homeowners etc.); and commercial insurance
(property insurance, general liability, worker’s compensation
etc.). We also offer life and health and risk management consulting
services.
Jesse Trevino Insurance Agency is committed
to protecting the confidentiality and security of any and
all information we may collect from you. This privacy statement
describes our information practices and policies.
A portion of the information we need
comes directly from you or your employees such as social security
numbers, addresses and phone numbers. In addition we may have
to find out more information about you or your organization
from other sources.
For example, life and health insurance
carrier applications often ask for “Protected Health
Information” (PHI). It is our policy and, in turn, the
policies of the insurance companies we represent to fully
comply with the Health Insurance Portability and Accountability
Act of 1996 (HIPAA). All employees of Jesse Trevino Insurance
Agency and the companies we represent that have access to
PHI must comply with their respective company’s detailed
privacy procedures.
If you are interested in obtaining
a personal lines policy, please note we will order standard
consumer reports from the motor vehicle department or other
independent reporting sources as part of processing your application.
We will obtain a record of accident claims and violations
on auto insurance and claims, home value and condition for
homeowners insurance. Information on credit history helps
us to secure the highest possible discounts for you. You have
the right to obtain certain items of information we have collected
about you, and to request correction of information if you
feel it is inaccurate.
Please be advised we treat everything
we know about you, your employees or your organization in
a strict, confidential manner. Our employees sign “Confidentiality
Agreements” as a condition of employment and take great
care in handling customer information.
We do not sell your personal information
to anyone. We do not share or distribute your personal information
to anyone except those third parties with whom we are working
to secure the best possible quote and/or product. Under no
circumstances will we disclose for marketing purposes any
medical information; information relating to a claim for benefit
or, a civil or criminal proceeding involving you; or personal
information relating to your character, personal habits, mode
of living or general reputation except as provided by law.
We take all reasonable precautions to safeguard your information
against loss, theft, and misuse, as well as unauthorized access,
disclosure, alteration and destruction.
When we disclose information to others to perform business
services for us, they are also required to take appropriate
steps to protect this information. They may only use this
information for the purposes of performing those business
services.
Any controversy or claim arising out
of or relating to our privacy policy, or the breach thereof,
shall be settled by arbitration in accordance with the rules
of the American Arbitration Association, and judgment upon
the award rendered by the arbitrator(s) may be entered in
any court having jurisdiction. |
